This case report describes a minimally invasive endodontic procedure of a tooth with calcific metamorphosis of the pulp, using ultrasonic activation, avoiding unnecessary removal of enamel and dentin. Calcific metamorphosis of the pulp is a frequent and common condition in teeth that have suffered trauma, and the endodontic treatment of these cases is considered a current challenge for clinical skills, therefore, it should be carefully studied and a conservative treatment plan should be carried out that minimizes the risk of iatrogenesis.
