PM-TOM (Personalized Medicine: Therapy Optimization Method) is a decision support tool designed to find treatments with the minimal STOPP and Beers criteria risks and ADRs caused by ADEs, DDIs, DCIs, DGIs and DFIs. The tool optimizes a treatment by considering drug classes selected by health professionals and applicable to the patient's conditions. The data set includes the details about the admission, discharge and optimized treatments and their comparison.
