Data of accidents causation of 39 HCAs investigation reports
Description
The data were collected to help confirm the probability matrix of the Bayesian network for human factor analysis and classification system(HFACs). The data file shows 39 hazardous chemical accidents in China, and the accidents causation of these accidents. The first six colums is the code of accidents, accident unit, accident area, accident date, accident type, and death toll of accidents, respectively. The residual columns are accident causation classifacations according to HFACs, in which 1 represents this reason exists, null represents this reson not exists. 29 accidents resports were collected from the official website of China Chemical Safety Association (http://www.chemicalsafety.org.cn/) and 10 were from the website of Baidu. Based on these data, the Bayesian network for HFACs was constructed, and the sensitivity of each node could be calculated. The results shown that human factor is the direct cause of operating errors, while hazardous material environment, mechanical equipment, and human factors are direct causes of operation violation. There is also the indirect cause of human unsafe behavior, they are the effects of organization climate, hidden danger investigation, organizational process on operation errors, and the effects of resource management, operation guidance, planned operation on operation violation. And compared with other factors, the sensitivity of organization climate is most obvious.

Steps to reproduce
39 cases were collected by website, which are the investigation reports of hazardous chemical accidents released by the government. Text analysis method was used in the transformation from investigation reports to variable codes. Taking the poisoning and asphyxiation accident of “Tianchang Bozi Manufacturing Co., Ltd. on May 23, 2008” as an example, according to the accident investigation report, the direct reason is that “operator Bi violated the relevant regulations of limited space operation and entered the limited space without wearing an air respirator and safety rope, resulting in inhaling hydrogen sulfide gas and death”. So the direct cause of the accident was judged as “operation violation”. And the indirect causes include “failure to fulfill the enterprise production safety responsibility ”, “confusion of safety management”, “failure to implement safety management and operation regulations”, “ formalize safety education”, “ weak awareness of safety”, “low ability of self-help and mutual rescue”, “inadequate management of labor protection equipment”, “serious deficiency of on-site management”, etc.
