Steps to reproduce

The code will not run directly on the provided data, but will require minor modifications that may be easy for those familiar with MATLAB. Instead, we provide the compiled clean data that are generated by the code, which you see is written out in ES_Combined. All identifying information (mostly dates, MRNs) have been removed or replaced by random identifiers.
