Columellar necrosis often results in injury to the overlying columellar skin, the medial crura of the lower lateral nasal cartilages, and the anteromedial inner nasal mucosa and possibly the anterior septal cartilage. The auricular chondrocutaneous composite graft is one of the best reconstructive methods to date. However, the risk of graft failure and deformity of the donor site increases with increasing graft size.
