Abstract Onychocryptosis is a pathology of the peronychium which, due to physical stress, causes hypertrophic growth of the germinal base of the phaneros, which results in an extremely painful dysmorphic nail plate, affecting the correct biomechanics of walking. A 19-year-old male patient, phototype 2, complaining of intense mycosis, pain, itching, and edema in the nails of both feet with 8 years of evolution, presenting severe chronic infection of the nail bed, phaneros in poor general condition, having traumatic lesions of real relevance. Due to the seriousness of the injuries, a treatment propaedeutic including antibiotics was chosen, even though the literature and practice do not recommend it, satisfying and accelerating the healing process. The Matricectomy procedure performed with chemical cauterization with ATA 80% resulted in morphologically normal growth, not showing a vitreous appearance. so we affirm that in certain cases the use of pharmacological agents with antibiotic and/or fungal action is necessary in lesions with evident chronicity.
