Steps to reproduce

Between July 15, 2021, and March 3, 2023, nine patients with type 2 diabetes were treated with oral semaglutide. Oral semaglutide was started with 3 mg once daily at least for 30 days, and then increased the dosage to 7 mg. We did not increase the dosage to 14 mg. The appropriate administration of oral semaglutide was confirmed by their partner. All patients visit our clinic every two months and HbA1c measurements were repeated. The prescriptions for type 2 diabetes were changed in patients who did not show any improvement in HbA1c with 7 mg oral semaglutide. A dose of 100 mg canagliflozin was added (No. 4), the medications were switched to 100 mg sitagliptin (No. 7), and to previous ones (No. 8) at 9, 10, and 8 months after oral semaglutide treatment, respectively. All patients also visited for medical checkup during follow-up period, and H. pylori antibodies were assessed if they wanted. Subsequently, the relationship between H. pylori infection and the effectiveness of oral semaglutide was studied. In H. pylori positive patients, the eradication therapy with 20 mg of vonoprazan, 750 mg of amoxicillin, and 200 mg of clarithromycin at a time, twice a day, was introduced for 7 days18. Successful eradication of H. pylori was defined to fulfill following criteria; Δ13C levels of less than 2.5 Δ‰ in the urea breath test assessed at least 30 days after eradication19, and more than 50% reductions of the titres of H. pylori antibodies evaluated at least 6 months after eradication, compared to those before eradication20. After eradication of H. pylori, oral semaglutide monotherapy was initiated with 7 mg prior to data acquisition of eradication outcomes, and then HbA1c was evaluated repeatedly. The maximum dosage of 14 mg was administered during oral semaglutide monotherapy whenever necessary (patients with two consecutive HbA1c% levels > 7.0%).
