Steps to reproduce

The package includes code and (publicly available) data used in the paper. In order to generate all tables and figures in the paper, the users need to obtain ALL data. The required data from subscription databases or from other authors are not included. Code for the use of the data to generate the results is included. Read the included pdf file with detailed instructions.
