Methods Study Population This study included 30 patients (aged >12 years) with early rheumatoid arthritis (symptom onset <1 year) presenting between March 2021 and January 2023. All patients underwent contrast-enhanced MRI (CE-MRI) and dual-energy CT (DECT) of the dominantly affected hand and wrist. Evaluation focused on the 2nd–5th MCP and PIP joints, carpal bones, and flexor tendons. MRI Protocol Imaging was performed on a 1.5T Philips Achieva system using a small surface coil. Sequences included coronal STIR, T1W, DWI, and mFFE; axial T2W SPAIR and T1W. Post-gadolinium (0.1 mmol/kg) T1W images were acquired in coronal and axial planes to assess synovial thickening, enhancement, bone marrow edema (BME), and tenosynovitis. DECT Protocol DECT was performed on a GE Revolution scanner in Gemstone Spectral Imaging (GSI) mode 60 seconds after IV contrast administration (100 ml at 3.5 ml/sec). Material density images were post-processed on an AW 2.0 workstation using iodine-water filters for iodine mapping and water-hydroxyapatite (HAP) filters for BME. Image Analysis Synovitis: Defined as thickening >2 mm; identified via MRI enhancement or DECT iodine-water map uptake. Tenosynovitis: Diagnosed by MRI signal/enhancement or DECT iodine uptake within the sheath. BME: Identified by MRI STIR signal or increased color on DECT water-HAP maps. 0 and 1 given on the basis of presence or absence Statistical Analysis Data were analyzed using STATA 16.1. Categorical variables were assessed via chi-square or Fisher’s exact tests. DECT diagnostic accuracy (sensitivity, specificity, PPV, NPV, and AUROC) was evaluated against MRI. Agreement was assessed using Cohen’s Kappa.
