Effect of sacroilliac joint manipulation on hip joint range of motion and muscle strength in normal healthy individuals

ABSTRACT Study Design: A single blinded, (Two group pre-test – post-test non randomized controlled trial). Purpose: To investigate the impact of sacroiliac joint (SIJ) manipulation on hip joint ranges of motion and muscle strength. Background: SIJ is fundamentally linked with lumbar spine and hip joint through thoracolumbar fascia and considered any dysfunction of the one component would affect the kinetic chain of lumbopelvic rhythm. The kinetic chain between lumbopelvic and hip complex coordinate during functional movement of hip joint. SIJ Manipulation stretches the shortened and thickened periarticular soft tissues which usually effect on the range of motion (ROM), improve the fluid drainage with in and around the joints, decrease the oedema and reduced the muscle spasm. The mechanical effect of the manipulation may restore bio mechanical stability to the involved motion segments by increasing strength and range of motion. There is lack of literature available that has evaluated the effect of sacroiliac joint manipulation on hip ranges of motion and muscle strength (MS). Methods and materials: A total of 50 asymptomatic subjects were recruited on the basis of selection criteria. Pre intervention assessment (Range of motion) was measured by universal goniometer and muscle strength was measured by baseline handheld dynamometer. On the same day after the intervention, post intervention assessment was noted. Results: Descriptive data was analyzed by using normality test. Paired t test was used to analyse within group comparison and Independent t- test was used to analyse between group comparisons. Within group comparison paired t test was applied at 95% of confident interval, that showed a statistically significant improvement in the range of motion (extension, external rotation, internal rotation) with p<0.05 for range of motion in both group. Muscle strength in both groups was also comparison, the statistically significant improvements in the all muscle strength (Flexors, extensors, abductors, adductors, internal rotators, and external rotators). For between group comparison Independent t test was applied at 95% of confident interval, that showed a statistically significant improvement in the range of motion (internal rotation) with p<0.05 for range of motion in both group comparison. Muscle strength in between group comparison, which was shown a statistically significant improvement in the muscle strength (adductors, external rotators, internal rotators). Conclusions: The study concluded that there was significance relation of sacroiliac joint manipulation with hip joint range of motion and muscle strength. We conclude that a single manipulation of the SIJ of students statistically improved the hip internal rotation range of motion and adductors, internal rotators, external rotators muscle strength.
