Autoantibodies in Pregnancy Loss

Recurrent pregnancy loss (RPL) is associated with antiphospholipid-related autoantibodies, but their clinical significance varies considerably across patients. We examined whether obesity affects this association, assessed their discriminatory performance across BMI groups, and assessed whether these associations differed across BMI strata. BMI-stratified analyses suggested heterogeneity in the association and discrimination of several antiphospholipid-related autoantibodies in RPL, but formal interaction testing was inconclusive. These findings support prospective validation of BMI-aware biomarker interpretation rather than immediate changes to diagnostic practice.

Steps to reproduce

All samples were analyzed within 6 months of collection, with no more than one freeze-thaw cycle. The selected antibody panel included both criteria-related and non-criteria antiphospholipid-associated antibodies implicated in RPL and thrombo-inflammatory complications of pregnancy. Serum samples were tested in duplicate using ELISA for aAnnV and aPZ IgM and IgG with Zymutest® kits (HYPHEN BioMed, Neuville-sur-Oise, France), as well as for aCL, aPS, aPT, and aβ2GPI IgM and IgG with REAADS kits (Corgenix, Broomfield, CO).
