Our study demonstrates a consistent inverse association between baseline fasting plasma glucose levels and both short- and long-term biologic effectiveness in patients with psoriasis, with the relationship extending linearly across the full glycemic spectrum. On the basis of these findings, we propose that glycemic monitoring and optimization should not be confined to psoriasis patients with comorbid diabetes, but rather extended to all individuals receiving biologic therapy—shifting the paradigm from selective intervention toward comprehensive metabolic management in this population.
