Based on the concept of “medication experience” proposed by Hillman et al., this study integrates qualitative evidence from both older patients and their family caregivers’ medication experience during transitions of care, aiming to offer a comprehensive and in-depth analysis of medication experience among elderly patients and family caregivers.
