倒刺线悬吊缝合技术在鼾症咽成形术中的应用效果研究 Study on the Application Effect of Barbed Suture Suspension Technique in Pharyngoplasty for Snoring

【摘要】目的:探讨倒刺线悬吊技术在鼾症咽成形术中的临床应用效果。方法:选取本院接受咽成形术的鼾症患者,随机分为观察组和对照组。观察组采用倒刺线悬吊技术,对照组采用传统缝合技术。比较两组手术时间、术中出血量、术后住院时间、伤口愈合时间、并发症发生率、术后疼痛程度、睡眠呼吸监测指标及生活质量改善情况。结果:观察组在手术时间、术中出血量、术后住院时间和伤口愈合时间方面均显著优于对照组(t =13.15, 8.25, 8.46, 8.54;P <0.05)。术后不同时间点VAS评分显示,观察组疼痛程度低于对照组(t=4.55, 3.18, 3.28;P<0.05)。术后多导睡眠图指标改善情况显示,观察组优于对照组(t=7.59, 6.93, 5.72;P<0.05)。术后匹兹堡睡眠质量指数评分也表明,观察组生活质量改善更明显(t=5.72, 6.38, 5.20, 7.67, 9.00;P<0.05)。观察组并发症发生率显著低于对照组(χ²=4.326,P<0.05)。结论:倒刺线悬吊技术在鼾症咽成形术中具有明显优势,可有效缩短手术时间、减少出血、减轻术后疼痛、促进伤口愈合、降低并发症,并显著改善患者的睡眠和生活质量。 Yan Li 1,Guangping BAI1, Yonggang JIN2* 1 Department of Otorhinolaryngology, Shanghai Yida Hospital, Shanghai University, Shanghai 201700, China 2 Department of Otorhinolaryngology, Xianghe County People's Hospital, Hebei Medical University, Langfang, Hebei 065499, China 【Abstract】 Objective: To evaluate the clinical efficacy of the barbed suture suspension technique in palatopharyngoplasty for the treatment of snoring. Methods: A total of 83 patients with snoring who underwent palatopharyngoplasty at our hospital were randomly assigned to either the observation group or the control group. The observation group received the barbed suture suspension technique, while the control group underwent traditional suture closure. The two groups were compared in terms of operative time, intraoperative blood loss, postoperative hospitalization duration, wound healing time, complication rate, postoperative pain levels (assessed using VAS scores), polysomnography parameters, and quality of life improvements. Results: The observation group showed significantly better outcomes than the control group in operative time, intraoperative blood loss, postoperative hospital stay, and wound healing time (t = 13.15, 8.25, 8.46, 8.54; P < 0.05). Postoperative VAS scores at different time indicated lower pain levels in the observation group (t = 4.55, 3.18, 3.28; P < 0.05). Polysomnography results at post-surgery demonstrated greater improvement in the observation group (t = 7.59, 6.93, 5.72; P < 0.05). Additionally, Pittsburgh Sleep Quality Index (PSQI) scores revealed more pronounced improvements in quality of life at multiple follow-up intervals in the observation group (t = 5.72, 6.38, 5.20, 7.67, 9.00; P < 0.05). The complication rate was also significantly lower in the observation group (χ² = 4.326, P < 0.05). Conclusion: The barbed suture suspension technique demonstrates significant clinical advantages in palatopharyngoplasty for snoring. It effectively reduces operative time and intraoperative bleeding, alleviates postoperative pain, accelerates wound healing, lowers complication rates, and leads to substantial improvements in sleep quality and overall patient quality of life.
