A representative scalp biopsy processed using the diagonal technique demonstrates a change in tissue contour from circular (A, 2x, H&E) to ovoid (B, 2x, H&E) for the superficial half of the punch biopsy, angled at time of embedding. The isthmus is visualized in an intermediate level (C, 4x, H&E). Elastic stain demonstrates loss of the perifollicular elastic network with adjacent elastic fiber thickening and recoil (D, 40x, elastic stain) supporting the diagnosis of scarring alopecia.
