Steps to reproduce
This file contains only Mathematica code and follows the same steps as outlined in the Online Appendix of the paper. The calculations in the Mathematica file are placed under headers relating each of them to a subsection within the Appendix. The Mathematica file starts from a Table of Contents that includes links to these headers within the file and thereby also provides an overview of which part of the file links to which part of the Appendix. Reproduction is immediate within the file: for any calculation coded in Mathematica, one can right-click and press "Evaluate Cell" to generate the response. Note: Mathematica software is required to open and use the file. Mathematica version 13.1 (most recent version as of January 2025) was used to create this file and no additional software or packages within the software have been used.
