This file contains a redacted and structured interpretation of historical safety incident records associated with biogas production and wastewater treatment operations in UK. The document was compiled as part of research investigating process safety risks, system failures, human and organisational factors, and risk-management practices within biogas facilities. The information is organised into three principal components: incident descriptions (“What Happened”), identified or investigated root causes, and corresponding corrective or preventive action plans. The records cover a range of process-safety events and operational concerns, including biogas releases and loss of containment, digester overpressure, water-seal and condensate-trap failures, sludge leaks, equipment and instrumentation failures, SCADA and control-system issues, pressure-relief events, maintenance deficiencies, inspection and monitoring weaknesses, and a steam-release incident. The root-cause information captures technical, operational, human and organisational contributors, including equipment condition and ageing, inadequate maintenance or inspection, instrumentation reliability, process design limitations, communication and control-system failures, training and supervision, incomplete implementation of previous corrective actions, and deficiencies in hazard identification and process-safety management. The action-plan information documents proposed or implemented risk-control measures, including improved inspection and maintenance programmes, instrumentation upgrades, enhanced monitoring and alarm systems, revised operating procedures, training, emergency preparedness, management-of-change controls, HAZOP review, improved incident reporting and learning, and engineering modifications. Information capable of directly identifying incident locations and personnel has been removed or anonymised for research-data sharing. The dataset is intended to support analysis of recurring process-safety risks, contributing factors, organisational learning and risk-mitigation strategies in biogas and wastewater-treatment operations.
